This is a four-drug combination regimen consisting of cetuximab, cisplatin, cixutumumab, and gemcitabine. Cetuximab is a chimeric monoclonal antibody targeting the epidermal growth factor receptor (EGFR), inhibiting downstream signaling pathways involved in cell proliferation and survival. Cisplatin is a platinum-based chemotherapeutic agent that induces DNA crosslinking and apoptosis. Cixutumumab is a fully human monoclonal antibody directed against the insulin-like growth factor 1 receptor (IGF-1R), blocking IGF-1-mediated signaling and promoting cancer cell apoptosis. Gemcitabine is a nucleoside analog that inhibits DNA synthesis by incorporation into DNA strands during replication, leading to cell death. This combination targets multiple oncogenic pathways—EGFR, IGF-1R, DNA replication/repair—and has been investigated for synergistic antitumor effects in solid tumors such as head and neck squamous cell carcinoma[6][8][10]. However, there are no reports of this exact four-drug regimen being approved or widely used; most clinical experience involves subsets of these agents.
