This combination therapy regimen consists of the EGFR-targeting monoclonal antibody cetuximab (Erbitux) and the GEMOX chemotherapy backbone, which includes gemcitabine (Gemzar) and oxaliplatin (Eloxatin). It was investigated in a Phase II clinical trial (NCT01216345) sponsored by the Association of Research on the Biology of Liver Tumors (ARBOL) as a first-line treatment for patients with unresectable locally advanced or metastatic biliary tract cancer, including cholangiocarcinoma and gallbladder cancer. Cetuximab works by binding to the extracellular domain of the epidermal growth factor receptor (EGFR), inhibiting downstream signaling pathways involved in cell proliferation and survival. Gemcitabine is a nucleoside analog that interferes with DNA synthesis, while oxaliplatin is a platinum-based agent that forms DNA cross-links, leading to apoptosis. The combination aims to provide synergistic antitumor activity in biliary tract malignancies, which frequently overexpress EGFR.
