Overview

Cetuximab + pembrolizumab is a combination of two monoclonal antibodies used in oncology. Cetuximab is an epidermal growth factor receptor (EGFR) inhibitor that blocks EGFR signaling and mediates antibody-dependent cellular cytotoxicity (ADCC), leading to inhibition of tumor cell proliferation and immune-mediated tumor cell death. Pembrolizumab is a programmed cell death protein 1 (PD-1) inhibitor that blocks the interaction between PD-1 on T cells and its ligands PD-L1/PD-L2, thereby enhancing T-cell mediated anti-tumor immunity. This combination has been investigated primarily for recurrent or metastatic head and neck squamous cell carcinoma (HNSCC), particularly in patients who are platinum-resistant or platinum-ineligible[1][2][4]. The rationale for combining these agents lies in their complementary mechanisms—direct tumor targeting by cetuximab with immune checkpoint blockade by pembrolizumab—to potentially enhance anti-tumor efficacy.
