Cetuximab + S-1 + cisplatin is a combination regimen consisting of cetuximab, a monoclonal antibody targeting the epidermal growth factor receptor (EGFR); S-1, an oral fluoropyrimidine derivative containing tegafur, gimeracil, and oteracil; and cisplatin, a platinum-based chemotherapeutic agent. The combination has been investigated as a first-line therapy for advanced gastric cancer and gastroesophageal junction adenocarcinoma. Cetuximab blocks EGFR-mediated signaling involved in tumor growth and survival, S-1 inhibits DNA synthesis in cancer cells, and cisplatin causes DNA crosslinking and apoptosis. In a phase 2 clinical trial, this regimen was well-tolerated in advanced gastric cancer, but adding cetuximab to S-1 plus cisplatin did not improve clinical benefit over S-1 plus cisplatin alone[1].
