Cetuximab + simvastatin is an investigational combination therapy consisting of cetuximab, a chimeric monoclonal antibody targeting the epidermal growth factor receptor (EGFR), and simvastatin, a small molecule HMG-CoA reductase inhibitor (statin). This combination has been studied primarily in metastatic colorectal cancer patients with KRAS mutations, who are typically resistant to cetuximab monotherapy. Preclinical and early clinical studies suggest that simvastatin may overcome cetuximab resistance by modulating BRAF activity and inducing apoptosis in KRAS mutant tumor cells. The rationale is based on statins' ability to inhibit post-translational modification of small G proteins such as Ras and Rho, thereby affecting downstream signaling pathways involved in tumor proliferation and survival[1][3][2]. Clinical trials have evaluated the safety and efficacy of adding simvastatin to standard cetuximab regimens in this patient population[4][2].
