Overview

**Cevostamab + lenalidomide** is a combination investigational therapy developed for multiple myeloma. **Cevostamab** is a bispecific monoclonal antibody that targets Fc receptor–homolog 5 (FcRH5) on myeloma cells and CD3 on T cells, thereby redirecting T cells to kill myeloma cells. It activates and induces expansion of CD8+ T cells and cytokine induction, notably IFN-γ[4][5][6]. **Lenalidomide** (brand name Revlimid) is an immunomodulatory agent that binds cereblon (CRBN), modulating the substrate specificity of the CRL4CRBN E3 ubiquitin ligase, resulting in the ubiquitination and proteasomal degradation of transcription factors (IKZF1, IKZF3) critical for myeloma survival, and casein kinase 1A1 (CK1α) in del(5q) myelodysplastic syndrome[2]. The combination is under clinical investigation as maintenance therapy post-transplant and for high-risk/relapsed multiple myeloma, aiming to leverage complementary immunomodulatory and T-cell redirecting mechanisms[1][3][5].
