CG-0255 Besylate is a novel small molecule prodrug of the active thiol metabolite of clopidogrel (Plavix), developed by Shanghai CureGene Pharmaceutical. Unlike clopidogrel, which requires a complex two-step hepatic metabolism primarily mediated by cytochrome P450 enzymes (such as CYP2C19) to generate its active form, CG-0255 is designed with an alternative conversion route. This mechanism is intended to bypass the genetic variability and potential drug-drug interactions associated with CYP2C19 metabolism, potentially providing more rapid and consistent platelet inhibition. The active metabolite irreversibly antagonizes the P2Y12 receptor on platelets, thereby preventing ADP-induced platelet aggregation. CG-0255 is being investigated in Phase 1 clinical trials in both oral (capsule) and intravenous (injection) formulations for the treatment and prevention of atherothrombotic events, including acute coronary syndrome, myocardial infarction, stroke, and peripheral arterial disease.
