ch14.18-IL2 is a combination immunotherapy composed of the chimeric anti-GD2 monoclonal antibody ch14.18 and interleukin-2 (IL-2), developed for the treatment of cancers such as high-risk neuroblastoma, melanoma, and sarcoma. ch14.18 binds the GD2 antigen on neuroblastoma and certain other cancer cells, facilitating immune-mediated tumor cell death by mechanisms including antibody-dependent cellular cytotoxicity (ADCC) and complement-dependent cytotoxicity (CDC). Interleukin-2 is a cytokine that stimulates proliferation and activation of immune effector cells, particularly natural killer (NK) cells and T cells. The combination is aimed at potentiating anti-tumor immunity by both targeting GD2-positive tumors and enhancing the cytotoxic immune cell response[2][4]. The combination has been evaluated in multiple phase I-III clinical trials for high-risk neuroblastoma and melanoma, demonstrating efficacy but substantial toxicities, including hypersensitivity reactions, capillary leak, fever, infection, and neuropathic pain[1][2][4][6][7].
