CHF6523 is an inhaled, selective phosphatidylinositol 3-kinase delta (PI3Kδ) inhibitor developed for the treatment of chronic obstructive pulmonary disease (COPD). It features a novel isocoumarin scaffold designed to ensure high selectivity for PI3Kδ and low systemic exposure by optimizing lung retention. Preclinical studies demonstrated durable anti-inflammatory activity in models of Th2-driven acute lung inflammation. In clinical trials, CHF6523 showed target engagement with significant reduction in induced sputum PIP₃ levels but did not translate into clear anti-inflammatory pharmacodynamic effects as measured by biomarkers and multi-omics analyses. The drug was generally well-tolerated; cough was the most common adverse event, typically mild or moderate and resolving quickly after dosing[1][2][4][7].
