CR-CHOP + PD1 inhibitor is an experimental combination regimen being evaluated for the treatment of newly diagnosed double-expressor diffuse large B-cell lymphoma (DEL DLBCL). The regimen consists of the CR-CHOP backbone—which combines the oral Class I and IIb histone deacetylase (HDAC) inhibitor chidamide (tucidinostat) with the standard-of-care R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone)—plus an unspecified PD-1 inhibitor. Chidamide acts as an epigenetic modulator to potentially sensitize lymphoma cells to chemotherapy and immunotherapy. The addition of a PD-1 inhibitor aims to overcome immune evasion in DEL DLBCL, a subtype characterized by MYC and BCL2 co-expression and poor prognosis. This regimen is currently being studied in a molecularly stratified Phase II trial (NCT06701357) sponsored by Ruijin Hospital.
