This combination regimen consists of chidamide, gemcitabine, cisplatin, and an immune checkpoint inhibitor, specifically a PD-1 monoclonal antibody. Chidamide is an oral histone deacetylase (HDAC) inhibitor that acts as an epigenetic modulator, potentially enhancing the antitumor activity of chemotherapy and immunotherapy by altering the tumor microenvironment and increasing the expression of tumor-associated antigens. Gemcitabine and cisplatin are standard cytotoxic chemotherapy agents that induce DNA damage and inhibit DNA synthesis. The addition of a PD-1 inhibitor aims to restore T-cell mediated anti-tumor immunity. This quadruple combination is being investigated as a first-line treatment for patients with advanced intrahepatic cholangiocarcinoma (ICC), a type of liver cancer, in a Phase II clinical trial led by Shanghai Zhongshan Hospital.
