Chlorazolamide-manganese(II) complex is a coordination complex consisting of the sulfonamide carbonic anhydrase inhibitor chlorazolamide (5-(2-chlorophenyl)-1,3,4-thiadiazole-2-sulfonamide) and manganese(II) ions. Developed as a potential treatment for glaucoma and ocular hypertension, this complex demonstrates significantly improved topical intraocular pressure (IOP) lowering activity compared to the parent sulfonamide in animal models. Its mechanism of action involves the potent inhibition of carbonic anhydrase isozymes, specifically CA I, CA II, and CA IV, which are involved in the secretion of aqueous humor in the eye. While the parent sulfonamide shows poor topical efficacy, the manganese complex significantly enhances the lowering of IOP, with ex vivo data showing nearly complete inhibition of CA II in ocular tissues.
