Overview

This is a combination of three antihypertensive and diuretic agents used together for the management of conditions such as resistant hypertension or severe fluid overload, particularly in patients with heart failure or renal impairment. - **Chlorothiazide** is a thiazide diuretic that inhibits sodium reabsorption in the distal tubules, increasing excretion of sodium and water along with potassium and hydrogen ions. - **Furosemide** is a potent loop diuretic that acts on the thick ascending limb of the loop of Henle to inhibit reabsorption of sodium and chloride, leading to increased urine output and reduction in fluid overload[2][5]. - **Hydralazine** is a direct vasodilator that relaxes arteriolar smooth muscle by interfering with calcium transport mechanisms, thereby reducing peripheral vascular resistance and lowering blood pressure[9]. The combination may be considered for patients who require aggressive blood pressure control or have refractory edema despite standard therapy. The use should be closely monitored due to risks such as electrolyte disturbances (notably hypokalemia), dehydration, hypotension, and potential drug interactions[5][8].
