Chlorthalidone + amiloride

Chlorthalidone + amiloride is a fixed-dose combination of two oral antihypertensive agents used primarily for the treatment of high blood pressure (hypertension) and prevention of fluid retention (edema). Chlorthalidone is a thiazide-like diuretic that inhibits sodium-chloride symporter in the distal convoluted tubule of the nephron, leading to increased excretion of sodium, chloride, and water[1][3][5]. Amiloride is a potassium-sparing diuretic that blocks epithelial sodium channel (ENaC) in the late distal tubules and collecting ducts, reducing sodium reabsorption while minimizing potassium loss[6]. The combination leverages chlorthalidone’s potent antihypertensive effect while counteracting its tendency to cause hypokalemia with amiloride’s potassium-sparing action. This combination has been shown to effectively reduce blood pressure and lower cardiovascular risk in patients with prehypertension or hypertension[2].
