chlorthalidone + bumetanide

This combination therapy consists of **chlorthalidone**, a thiazide-like diuretic, and **bumetanide**, a loop diuretic. It is being investigated by the **Hospital General de México Dr. Eduardo Liceaga** in the **HEBE-CKD Trial** (NCT03923933) specifically for patients with advanced chronic kidney disease (CKD stages 4-5) who suffer from hypertension and volume overload. The rationale behind the combination is to overcome 'diuretic resistance'—a common phenomenon in advanced CKD where the distal nephron undergoes adaptive changes to compensate for loop diuretic action. By simultaneously inhibiting the sodium-potassium-chloride cotransporter in the loop of Henle (via bumetanide) and the sodium-chloride cotransporter in the distal convoluted tubule (via chlorthalidone), the therapy achieves a more potent natriuretic effect and better volume control than monotherapy.
