10% cholesterol cream is a topical therapeutic formulation developed by the Institute of Dermatology, Thailand, for the treatment of autosomal recessive congenital ichthyosis (ARCI). ARCI is a rare genetic skin disorder characterized by a defective skin barrier due to impaired production or organization of natural lipids, such as cholesterol, ceramides, and fatty acids. This deficiency leads to increased transepidermal water loss (TEWL), resulting in severe dryness, scaling, and pruritus. Cholesterol is a critical physiological component of the stratum corneum's lamellar lipid matrix. By applying cholesterol topically at a high concentration, the treatment aims to replenish the lipid structure, restore barrier function, and improve skin hydration and patient quality of life.
