Overview

CHOP-L is a combination chemotherapy regimen, most likely referring to the standard CHOP regimen (cyclophosphamide, doxorubicin, vincristine, and prednisone/prednisolone) with the addition of L-asparaginase. This modification is commonly used in certain lymphomas such as extranodal NK/T-cell lymphoma. The mechanism of action involves multiple cytotoxic effects: - Cyclophosphamide is an alkylating agent that cross-links DNA. - Doxorubicin (hydroxydaunorubicin) intercalates into DNA and inhibits topoisomerase II. - Vincristine binds tubulin and inhibits microtubule formation. - Prednisone/prednisolone are corticosteroids that induce apoptosis in lymphoid cells. - L-asparaginase depletes asparagine, an amino acid necessary for protein synthesis in some cancer cells. This combination targets rapidly dividing malignant lymphocytes through several complementary mechanisms. It is primarily indicated for aggressive non-Hodgkin lymphoma subtypes including T-cell and NK/T-cell lymphomas[6].
