Overview

Chronic Intermittent Intravenous Insulin Therapy (CIIIT) is a specialized therapeutic protocol developed to treat microvascular complications in patients with Type 1 diabetes. Developed and studied by the Joslin Diabetes Center, the therapy involves the administration of pulsed intravenous insulin infusions—typically three one-hour courses over an eight-hour period once per week—in addition to a standard subcutaneous insulin regimen. This pulsatile delivery is intended to mimic the natural physiological secretion of insulin into the portal vein, thereby optimizing hepatic glucose metabolism and correcting the respiratory quotient. The primary goal of CIIIT is to slow or prevent the progression of diabetic nephropathy and diabetic retinopathy by improving overall glycemic control and metabolic efficiency.
