Cibinetide is a synthetic 11-amino acid peptide engineered to mimic the helix B surface domain of erythropoietin (EPO), but unlike EPO, it does not stimulate red blood cell production. Instead, cibinetide selectively binds and activates the innate repair receptor (IRR), a heterodimer composed of the erythropoietin receptor (EPOR) and the cytokine family beta common receptor (CD131). Activation of IRR triggers anti-inflammatory and tissue-protective pathways without hematopoietic side effects. Cibinetide has demonstrated potential in promoting nerve regeneration, reducing pain in neuropathies such as sarcoidosis-associated small fiber neuropathy, and decreasing retinal leakage in diabetic macular edema. It is being developed primarily for orphan indications including sarcoidosis-related neuropathy and diabetic macular edema[2][4][5][6][7][8].
