Overview

Cibisatamab is a humanized IgG1 bispecific monoclonal antibody designed as a T-cell engager for cancer immunotherapy. It simultaneously binds to carcinoembryonic antigen (CEA) on tumor cells and the CD3 epsilon chain on T cells. This dual binding redirects and activates cytotoxic T cells to target and kill CEA-expressing tumor cells independently of their native antigen specificity. The drug is optimized with a 2-to-1 molecular format (two CEA binding sites for avidity tuning), which enhances selectivity for cancer cells with moderate to high surface expression of CEA while sparing normal tissues with low expression. Developed primarily by Roche/Genentech, cibisatamab has been investigated in advanced solid tumors—especially microsatellite stable colorectal cancer (MSS-CRC)—as monotherapy or in combination with other immunotherapies such as atezolizumab after obinutuzumab pre-treatment[1][2][4][6][7]. Clinical trials have shown preliminary antitumor activity but also significant rates of anti-drug antibodies and immune-related adverse events.
