Cibotercept is a recombinant fusion protein developed by Keros Therapeutics. It consists of a modified ligand-binding domain of the activin receptor type IIB (ActRIIB) fused to the Fc domain of human immunoglobulin. Cibotercept acts as an inhibitor of transforming growth factor beta (TGF-β) ligands, specifically targeting activin A, activin B, and myostatin. By inhibiting these ligands, it is designed to modulate signaling pathways involved in smooth muscle hypertrophy and fibrosis and potentially increase bone morphogenic protein (BMP) pathway signaling. The drug has been investigated for several indications including pulmonary arterial hypertension (PAH), chronic heart failure, osteoporosis, and osteogenesis imperfecta. Development in PAH was discontinued due to safety concerns related to pericardial effusion[1][2][3][5][6].
