Drug intelligence / Profile preview
CicloMulsion is a Cremophor-free, lipid emulsion formulation of the small molecule cyclosporine A, originally developed by NeuroVive Pharmaceutical (now Abliva). In the context of the CLOTILDE trial led by Hospices Civils de Lyon, it was investigated for its potential to reduce myocardial reperfusion injury in patients with acute myocardial infarction (AMI) complicated by cardiogenic shock. Unlike its traditional role as an immunosuppressant (which involves calcineurin inhibition), its cardioprotective mechanism focuses on binding to cyclophilin D (peptidyl-prolyl cis-trans isomerase F) within the mitochondria. This binding inhibits the opening of the mitochondrial permeability transition pore (mPTP), a critical event in cell death during reperfusion. The formulation is designed for intravenous bolus administration during the reperfusion phase of treatment. The CLOTILDE trial was ultimately terminated following negative results from the larger CIRCUS trial evaluating cyclosporine in STEMI patients.
