CIGB-500 is a synthetic hexapeptide cytoprotective agent developed by the Cuban Center for Genetic Engineering and Biotechnology (CIGB). It is a derivative of growth hormone-releasing peptide-6 (GHRP-6) that exhibits potent cardioprotective properties. The drug is primarily indicated for the treatment of acute myocardial infarction and the prevention of ischemia-reperfusion injury. Its mechanism of action involves binding to the growth hormone secretagogue receptor (GHSR-1a) and the CD36 scavenger receptor, which modulates inflammatory responses, reduces oxidative stress, and inhibits apoptosis in cardiomyocytes. Phase 1 and Phase 1b clinical trials have confirmed the safety and tolerability of CIGB-500 in healthy volunteers and patients with acute myocardial infarction.
