CII-synNotch T cells are an engineered cell therapy designed for the targeted treatment of osteoarthritis (OA). These T cells express a synthetic Notch (synNotch) receptor featuring an extracellular recognition domain (ERD) derived from a monoclonal antibody (mAbCII) that specifically recognizes type II collagen (CII) epitopes exposed only in degraded articular cartilage. Upon binding to CII, the synNotch receptor undergoes proteolytic cleavage, releasing a synthetic transcription factor (intracellular domain) that translocates to the nucleus to drive the expression of therapeutic payloads. This system allows for spatially constrained, disease-specific transgene production, potentially reducing off-target effects compared to constitutive expression. The platform has been optimized using a single-chain fragment antigen-binding (scFab) architecture to enhance activation in T cells.
