CIL001 is a novel disease-modifying drug candidate developed for the treatment of cochlear synaptopathy, also known as "hidden hearing loss." This condition is characterized by impaired speech intelligibility in noisy environments and is an early sign of age-related hearing loss. Cochlear synaptopathy results from damage to the synapses connecting inner hair cells to the auditory nerve and can progress irreversibly to deafness if untreated. CIL001 has demonstrated preclinical efficacy in restoring auditory nerve activity and repairing cochlear synapses, as evidenced by improvements in the amplitude of the first wave of the auditory brainstem response (ABR) and reconnection of cochlear synapses. The drug is administered via a single injection through the tympanic membrane. It holds a comprehensive safety dossier and has advanced directly into phase 2a clinical trials targeting patients with type 2 diabetes who have cochlear synaptopathy. Originally licensed from Sanofi, Cilcare leads its development. Mechanism of action details are not fully disclosed; however, preclinical data indicate that CIL001 enhances gene expression necessary for synaptic recovery, increases cochlear synapse numbers, and improves ABR Wave I amplitude across frequencies—suggesting direct repair or protection at inner ear neural connections.
