Ciltacabtagene autoleucel + idecabtagene vicleucel is a hypothetical or investigational sequential combination of two BCMA-directed CAR T-cell therapies for relapsed/refractory multiple myeloma (RRMM). Ciltacabtagene autoleucel (cilta-cel, developed by Janssen/Legend Biotech) uses two single-domain antibodies targeting B-cell maturation antigen (BCMA) linked to 4-1BB and CD3-zeta domains, while idecabtagene vicleucel (ide-cel, developed by Bristol Myers Squibb) employs a single-domain BCMA binder with the same signaling domains; both are autologous T cells genetically modified ex vivo to express these CARs, enabling recognition and elimination of BCMA-expressing malignant plasma cells. No clinical data exists for their direct combination as a single product; studies compare them separately, showing cilta-cel often superior in PFS/OS but with higher severe CRS/ICANS risk in triple-class exposed RRMM patients previously treated with proteasome inhibitors, immunomodulators, and anti-CD38 antibodies.[1][3][4][6]
