CIML NK cells + aldesleukin is an investigational adoptive cellular immunotherapy regimen developed at the Washington University School of Medicine for the treatment of relapsed or refractory acute myeloid leukemia (AML) and myelodysplastic syndromes (MDS). The approach involves the ex vivo activation of haploidentical donor-derived natural killer (NK) cells with a cytokine cocktail consisting of interleukin-12 (IL-12), IL-15, and IL-18 for 12-16 hours. This process induces a 'cytokine-induced memory-like' (CIML) phenotype, characterized by enhanced proliferation, longevity, and potent cytotoxic activity against myeloid leukemia cells. Following lymphodepleting chemotherapy, patients receive the CIML NK cell infusion followed by low-dose subcutaneous aldesleukin (recombinant IL-2) to support the expansion and persistence of the donor cells in the host environment.
