This combination therapy regimen consists of the calcimimetic cinacalcet and active vitamin D sterols (such as calcitriol, paricalcitol, or doxercalciferol), primarily used for the management of secondary hyperparathyroidism (SHPT) in patients with chronic kidney disease (CKD) on dialysis. Cinacalcet acts as a positive allosteric modulator of the extracellular calcium-sensing receptor (CaSR) on the parathyroid gland, increasing its sensitivity to extracellular calcium and thereby inhibiting the secretion of parathyroid hormone (PTH). Active vitamin D sterols function as agonists at the vitamin D receptor (VDR), which directly suppresses PTH gene expression. The combination is clinically significant because cinacalcet tends to lower serum calcium and phosphorus levels, whereas vitamin D sterols tend to raise them; using them together allows for potent PTH suppression while maintaining better mineral balance and minimizing the hypercalcemia often associated with high-dose vitamin D monotherapy.
