Cinacalcet + vitamin D analogue is a combination therapy used primarily for the management of secondary hyperparathyroidism (SHPT) in patients with chronic kidney disease on dialysis. Cinacalcet is a calcimimetic agent that increases the sensitivity of the calcium-sensing receptor on parathyroid cells, thereby reducing parathyroid hormone (PTH) secretion. Vitamin D analogues (such as calcitriol, alfacalcidol, or paricalcitol) act by binding to the vitamin D receptor and suppressing PTH synthesis while promoting calcium absorption from the gut. The combination aims to achieve better control of PTH levels and mineral metabolism than either agent alone. Clinical studies suggest that this combination can improve biochemical parameters such as blood calcium compliance rate and may allow for lower doses of each component; however, it does not consistently improve survival compared to monotherapy and may increase certain adverse effects like hypocalcemia or gastrointestinal symptoms[1][2][4][6][7].
