Cinoxacin is a synthetic antibacterial agent belonging to the first-generation quinolone class. It was primarily used for the prevention and treatment of initial and recurrent urinary tract infections (UTIs) in adults, especially those caused by susceptible gram-negative bacteria such as Escherichia coli, Proteus mirabilis, Proteus vulgaris, Klebsiella species (including K. pneumoniae), and Enterobacter species[3][4][8]. Its mechanism of action involves inhibiting bacterial DNA gyrase (a type II topoisomerase) and topoisomerase IV, enzymes essential for DNA replication and cell division; this results in inhibition of bacterial DNA synthesis and is bactericidal[4][5][6][7]. Cinoxacin achieves high urinary concentrations after oral administration but has been discontinued in many countries due to the availability of newer agents with improved safety profiles[5]. The drug was developed by Eli Lilly.
