Cinrebafusp alfa (PRS-343) is a bispecific fusion protein designed for immuno-oncology applications, particularly in HER2-positive cancers. It consists of a 4-1BB-targeting Anticalin protein fused to a HER2-targeting antibody, enabling the drug to bridge and cluster T cells at the site of HER2-expressing tumor cells. This localizes 4-1BB activation within the tumor microenvironment, enhancing T cell proliferation and cytotoxic activity while minimizing systemic toxicity. Cinrebafusp alfa has demonstrated clinical benefit and an acceptable safety profile in patients with previously treated HER2-positive malignancies, including gastric cancer. The drug has received orphan drug designation for gastric cancer and is being evaluated in phase II trials for this indication[3][4][6][7].
