Overview

The combination of cisplatin, filgrastim, and fluorouracil is a chemotherapy regimen used primarily in cancer treatment. This combination includes a platinum-based chemotherapeutic agent (cisplatin), a granulocyte colony-stimulating factor (filgrastim), and an antimetabolite (fluorouracil or 5-FU). ## Mechanism and Usage This combination is used in various cancer treatments, particularly for esophageal cancer and malignant germ cell tumors. Cisplatin and fluorouracil have been shown to possess synergistic cytotoxic effects against various human tumors[5]. Cisplatin is believed to enhance the antitumor effect of fluorouracil by increasing the availability of reduced folate necessary for tight binding of fluorodeoxyuridylate (a fluorouracil metabolite) to deoxythymidylic acid synthase[5]. Filgrastim (G-CSF) is administered as supportive care to prevent or reduce the risk of febrile neutropenia, a common and potentially serious side effect of the chemotherapy combination[1][2][3]. It works by stimulating the bone marrow to produce white blood cells, particularly neutrophils. ## Administration Protocol The typical administration protocol involves: - Cisplatin: Intravenous administration (often 70 mg/m² per day on day 1)[1][2] - Fluorouracil (5-FU): Continuous infusion (often 750 mg/m² per day on days 1-5)[1][2] - Filgrastim: Subcutaneous injection, with timing varying by protocol: - In some regimens, pegfilgrastim (a long-acting form of filgrastim) is given as a single 3.6 mg injection on day 3[1][2] - In other protocols, filgrastim is administered at 5 μg/kg/day on specific days after chemotherapy (e.g., days 3-9 or days 6-19, depending on the specific regimen)[3] The treatment cycle is typically repeated every 3 weeks for up to three courses[1][2]. ## Efficacy and Concerns Studies have shown that the routine use of filgrastim significantly improved the delivery of planned chemotherapy schedules[3]. However, research has also indicated that filgrastim alone may not be sufficient to prevent febrile neutropenia in all cases, and prophylactic antibiotics may also be required for optimal protection[1][2]. In one study of patients with malignant germ cell tumors, 85% of patients randomized to receive filgrastim completed at least six chemotherapy cycles compared with 70% of patients who did not receive filgrastim[3]. Patients in the filgrastim arm achieved significantly higher dose-intensities and experienced fewer toxic deaths associated with febrile neutropenia[3]. ## Side Effects Common side effects of this combination therapy include: ### Hematological Effects - Neutropenia (including grade 4 neutropenia)[1][2] - Febrile neutropenia[1][2][3] - Increased risk of infection[9] - Anemia (looking pale and breathlessness)[9] - Increased risk of bleeding and bruising[9] ### Gastrointestinal Effects - Diarrhea[6][8][9] - Nausea and vomiting (can be severe)[6][8][9] - Loss of appetite[6][8] - Heartburn[6][8] - Mouth sores[6][8] - Stomach cramps[6][8] ### Other Effects - Fatigue/weakness[6][8][9] - Skin rash and itching[6][8] - Hair loss (temporary)[6][8] - Black, tarry stools[6][8] - Cough or hoarseness (especially with fever or chills)[6][8] - Lower back or side pain[6][8] - Painful or difficult urination[6][8] More serious but rare side effects can include chest pain, shortness of breath, and unusual bleeding or bruising[6][8]. The combination of cisplatin, filgrastim, and fluorouracil represents an important treatment approach for certain cancers, with filgrastim playing a crucial role in managing the hematological toxicities associated with the chemotherapy agents.
