The combination of cisplatin, doxorubicin, and cyclophosphamide—commonly referred to as the CAP regimen—is a multi-agent chemotherapy protocol used primarily in the treatment of advanced or metastatic cancers such as salivary gland carcinomas and adenocarcinoma of unknown primary site. - **Cisplatin** is a platinum-based chemotherapeutic that forms DNA crosslinks, leading to apoptosis by inhibiting DNA synthesis and function. - **Doxorubicin** is an anthracycline antibiotic that intercalates into DNA, inhibits topoisomerase II, and generates free radicals causing cytotoxicity. - **Cyclophosphamide** is an alkylating agent that crosslinks DNA strands, preventing cell division and leading to cell death. This combination exploits different mechanisms of action for synergistic antitumor effects but is associated with significant toxicity (notably myelosuppression, nausea/vomiting, anemia) and is generally reserved for palliative management or when other treatments are not feasible[1][2][3][5].
