Overview

This is a multi-agent chemotherapy regimen combining four cytotoxic drugs—cisplatin, epirubicin, etoposide, and ifosfamide—primarily used for the treatment of non-small cell lung cancer (NSCLC), both limited-stage and extensive-stage disease. The regimen has been evaluated in phase I/II clinical trials for its feasibility and activity. The mechanism of action involves DNA damage induction through different pathways: cisplatin forms DNA crosslinks; epirubicin intercalates into DNA and inhibits topoisomerase II; etoposide inhibits topoisomerase II leading to DNA strand breaks; ifosfamide alkylates DNA bases causing strand breaks[1]. This combination is typically administered intravenously in cycles.
