Overview

SIMPLE is a combination chemotherapy regimen developed by The University of Hong Kong for the treatment of natural killer (NK) cell malignancies, specifically extranodal NK/T-cell lymphoma and aggressive NK leukaemia. The regimen is a modification of the earlier PIGLETS protocol, renamed to avoid religious insensitivity and updated to include the neurokinin-1 receptor antagonist aprepitant to better manage chemotherapy-induced nausea and vomiting. SIMPLE consists of cisplatin, gemcitabine, ifosfamide, etoposide, L-asparaginase, and dexamethasone. It is currently being evaluated in a Phase 3 non-inferiority trial against the SMILE regimen, aiming to provide a comparable therapeutic effect with a lower risk of nephrotoxicity.
