This combination therapy is a triplet chemotherapy regimen being evaluated by Fudan University for the treatment of pancreatic cancer, specifically in patients with peritoneal metastasis. The protocol utilizes a regional delivery approach where cisplatin is administered intraperitoneally to achieve high local concentrations in the peritoneal cavity to target peritoneal seeding, while gemcitabine and nab-paclitaxel are administered intravenously for systemic control. Cisplatin is a platinum-based small molecule that acts as a DNA alkylating agent, inducing intra-strand and inter-strand cross-links that lead to apoptosis. Gemcitabine is a pyrimidine antimetabolite that inhibits DNA synthesis by targeting ribonucleotide reductase and competing for incorporation into DNA. Nab-paclitaxel is a nanoparticle albumin-bound formulation of paclitaxel that stabilizes microtubules, preventing their disassembly and causing cell cycle arrest at the G2/M phase.
