Overview

This is an investigational combination chemotherapy regimen developed by the National Cancer Institute (NCI) for the treatment of peritoneal carcinomatosis. The protocol involves cytoreductive surgery followed by Continuous Hyperthermic Peritoneal Perfusion (CHPP) with cisplatin, and subsequent early postoperative intraperitoneal (IP) administration of paclitaxel and 5-fluorouracil (5-FU). Cisplatin acts as a platinum-based DNA cross-linking agent that inhibits DNA replication. Paclitaxel is a taxane that stabilizes microtubules, preventing their disassembly and thereby arresting the cell cycle. 5-fluorouracil is a pyrimidine analog that inhibits thymidylate synthase, disrupting the synthesis of thymidine monophosphate and leading to DNA damage. This multi-drug approach is designed to target residual microscopic disease within the peritoneal cavity following surgical debulking.
