Overview

**Cisplatin + pembrolizumab + rintatolimod** is an investigational triplet combination therapy primarily evaluated in a phase II clinical trial (NCT05756166 or related NCI-2019-02783) for recurrent ovarian cancer and potentially other advanced solid tumors. **Cisplatin**, a platinum-based small molecule chemotherapeutic, forms DNA crosslinks to induce apoptosis in rapidly dividing cancer cells. **Pembrolizumab**, a high-affinity humanized IgG4 monoclonal antibody, binds PD-1 to block inhibitory signaling, thereby reactivating tumor-specific T-cell immunity. **Rintatolimod** (Ampligen), a synthetic mismatched double-stranded RNA (poly I:poly C12U), acts as a selective TLR3 agonist to trigger innate immune activation via the TRIF pathway, inducing type I interferons, chemokines, and antiviral state without strong pro-inflammatory cytokine release via MyD88. The combination leverages chemotherapy-induced immunogenic cell death, PD-1 blockade for adaptive immunity, and TLR3 agonism for enhanced innate responses, showing promising interim results including 45% ORR and 7.8-month median PFS in platinum-sensitive recurrent ovarian cancer patients.[2][5]
