This combination is often part of multi-drug regimens for treating different types of cancers, particularly gastrointestinal cancers and germ cell tumors. ## Medical Uses The cisplatin + epirubicin combination is used in several treatment regimens: 1. **ECX (Epirubicin, Cisplatin, Capecitabine)** - Used for: - Stomach cancer - Esophageal (food pipe) cancer - Gastroesophageal junction cancer 2. **CIS-EPI (Cisplatin-Epirubicin)** - Used for: - Metastatic germ cell tumors not amenable to cure with standard salvage therapy 3. **ECU (Epirubicin, Cisplatin, UFT)** - Used for: - Metastatic gastric cancer 4. **PEV (Cisplatin-Epirubicin-Vinorelbine)** - Used for: - Breast cancer ## Administration The administration of cisplatin + epirubicin varies depending on the specific regimen: - Both drugs are typically administered intravenously - Cisplatin is often given at 20 mg/m² on days 1-5 of a cycle - Epirubicin dosage ranges from 50-90 mg/m² on day 1 of a cycle - Treatment cycles are typically repeated every 3 weeks ## Efficacy Clinical trials have shown promising results: - In metastatic germ cell tumors, the CIS-EPI regimen achieved complete remission in 9 of 30 patients, with 7 patients remaining disease-free at 25+ to 48+ months - The ECU regimen for metastatic gastric cancer showed a 25% partial response rate with a tumor control rate of 53% - Median time to progression and overall survival with ECU were 6 and 8.6 months, respectively ## Side Effects Common side effects include: **Hematologic toxicity:** - Neutropenia (including neutropenic fever) - Thrombocytopenia - Anemia **Non-hematologic toxicity:** - Acute renal failure - Electrolyte imbalances - Nausea/vomiting - Elevated liver enzymes - Diarrhea Rare side effects (less than 1% of patients) may include: - Secondary cancers such as leukemia - Seizures - Increased skin sensitivity to sunlight - Inflammation or blockage in the bowel - Memory changes or confusion The combination has been noted to have an acceptable toxicity profile without significant mucositis, cardiotoxicity, or treatment-related deaths in some studies.
