CITA-RNPT (Continuous In-situ Ultra High-dose Antibiotics and Regulated Negative Pressure-assisted Wound Therapy) is a therapeutic intervention developed at Hillel Yaffe Medical Center for the salvage of infected cardiovascular implantable electronic devices (CIEDs). The protocol involves minimally invasive surgery (MIS) to access the infected pocket, followed by the continuous localized delivery of ultra-high-dose antibiotics (CITA) directly to the site of infection. This pharmacological treatment is integrated with regulated negative pressure-assisted wound therapy (RNPT), which manages wound exudate, promotes tissue healing, and maintains a controlled environment for the high-dose antibiotic concentration at the device-tissue interface. This approach is intended as a less invasive alternative to the standard total extraction of the CIED system, which is associated with significant procedural risks.
