This is a non-standard, theoretical combination of four selective serotonin reuptake inhibitors (SSRIs): citalopram, escitalopram, fluoxetine, and sertraline. Each of these agents is an SSRI antidepressant approved individually for the treatment of major depressive disorder and various anxiety disorders. All four drugs act primarily by inhibiting the reuptake of serotonin (5-HT) into presynaptic neurons in the central nervous system, thereby increasing synaptic serotonin levels and enhancing serotonergic neurotransmission[1][2][3][4][5]. Citalopram and escitalopram are closely related; escitalopram is the S-enantiomer (active component) of racemic citalopram[7]. Fluoxetine and sertraline are also widely used SSRIs with similar mechanisms but distinct pharmacokinetic profiles. Combining multiple SSRIs is not standard clinical practice due to a high risk for serious adverse effects such as serotonin syndrome—a potentially life-threatening condition caused by excessive serotonergic activity[6][8]. This combination would be expected to have additive or synergistic effects on serotonin reuptake inhibition but also greatly increase risks for side effects including agitation, gastrointestinal disturbances, sexual dysfunction, QT prolongation (especially from citalopram), drug interactions via cytochrome P450 inhibition (notably with fluoxetine), and most importantly severe toxicity such as serotonin syndrome[6][8].
