Citra-Lock is a concentrated trisodium citrate solution used as a catheter lock solution for patients undergoing chronic hemodialysis. It is designed to be instilled into the lumens of central venous catheters between dialysis sessions to maintain patency and reduce the risk of infection. Citra-Lock works via a dual mechanism: it acts as a potent anticoagulant by chelating ionized calcium, which is a critical cofactor in the coagulation cascade, and it provides antimicrobial properties through its hypertonicity and alkaline pH, which create an environment inhospitable to bacterial growth. Compared to traditional heparin locks, Citra-Lock is associated with a lower risk of systemic bleeding and heparin-induced thrombocytopenia (HIT), while maintaining similar or superior efficacy in preventing catheter-related bloodstream infections (CRBSI) and thrombotic occlusions.
