Citrasate is a citric acid-based dialysate solution developed by Advanced Renal Technologies for use in hemodialysis. It is designed to provide regional anticoagulation within the dialysis circuit by replacing the acetic acid typically found in standard bicarbonate dialysate with a low concentration of citric acid (citrate). The citrate acts as a chelating agent that binds to ionized calcium in the extracorporeal blood circuit; since calcium is a critical cofactor in the coagulation cascade, its sequestration prevents blood clotting within the dialyzer. This regional effect allows for effective dialysis without the need for systemic anticoagulation, thereby reducing the risk of systemic bleeding. Citrasate is used in both chronic and acute renal failure settings, including in critically ill patients with stage III acute kidney injury undergoing sustained low efficiency daily hemodialysis (SLEDD).
