Overview

Cladribine + idarubicin + cytarabine + tyrosine kinase inhibitor is an intensive combination chemotherapy regimen, often referred to by the acronym CLIA-TKI, primarily developed and studied by the MD Anderson Cancer Center. The regimen utilizes a backbone of three small molecule cytotoxic agents: cladribine (a purine nucleoside analog), idarubicin (an anthracycline), and cytarabine (a pyrimidine nucleoside analog). To this backbone, a tyrosine kinase inhibitor (TKI) is added to target specific molecular drivers of leukemogenesis. In patients with FLT3-mutated acute myeloid leukemia (AML), TKIs such as midostaurin, gilteritinib, sorafenib, or quizartinib are incorporated. In patients with myeloid blast phase chronic myeloid leukemia (CML-MBP), BCR-ABL1 inhibitors like ponatinib, dasatinib, or bosutinib are used. The regimen is designed to provide potent cytoreduction while simultaneously inhibiting signaling pathways that promote cancer cell survival and proliferation, serving as a bridge to allogeneic stem cell transplantation in high-risk hematologic malignancies.
