CLAG + FLAG + IDAC refers to a group of intensive chemotherapy (IC) regimens used in the treatment of myeloid malignancies, particularly relapsed or refractory acute myeloid leukemia (AML) and myelodysplastic syndrome (MDS). This protocol grouping includes CLAG (cladribine, cytarabine, and filgrastim), FLAG (fludarabine, cytarabine, and filgrastim), and IDAC (intermediate-dose cytarabine). These regimens are designed to maximize the cytotoxic effects of cytarabine by using purine analogs (cladribine or fludarabine) to increase the intracellular concentration of the active cytarabine metabolite, Ara-CTP. Additionally, filgrastim (G-CSF) is administered to prime leukemic blasts, increasing their sensitivity to the antimetabolite therapy. These intensive approaches are typically employed as salvage therapy or as a bridge to allogeneic stem cell transplantation.
