Overview

This is a four-drug combination therapy consisting of clarithromycin, metronidazole, amoxicillin, and omeprazole. It is used as a quadruple regimen for the eradication of Helicobacter pylori (H. pylori) infection and for the treatment and prevention of peptic ulcers associated with H. pylori. - **Clarithromycin** is a macrolide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit. - **Metronidazole** is a nitroimidazole antibiotic effective against anaerobic bacteria and protozoa; it disrupts DNA synthesis in susceptible organisms. - **Amoxicillin** is an aminopenicillin-type beta-lactam antibiotic that inhibits bacterial cell wall synthesis. - **Omeprazole** is a proton pump inhibitor (PPI) that suppresses gastric acid secretion by inhibiting the H+/K+ ATPase enzyme system at the secretory surface of gastric parietal cells. The rationale behind this combination therapy lies in using multiple antibiotics to overcome resistance patterns and improve eradication rates while reducing ulcer recurrence. Omeprazole enhances antibiotic efficacy by increasing gastric pH, which improves stability and activity of antibiotics against H. pylori[3][1]. This quadruple regimen has demonstrated high effectiveness (over 90% eradication rate in some studies) with good tolerability[3].
