Overview

Claudin18.2 (CLDN18.2) CAR-T cells represent a class of adoptive cell therapies designed to target and eliminate cancer cells expressing the Claudin18.2 protein. CLDN18.2 is a tight junction protein found to be highly expressed in various malignant tumors, including gastric, pancreatic, and ovarian cancers, while exhibiting limited presence in normal healthy tissues, making it an attractive therapeutic target. This therapy involves genetically modifying a patient's own T cells to express chimeric antigen receptors (CARs) that specifically recognize and bind to CLDN18.2 on the surface of cancer cells. Upon binding, these engineered CAR-T cells become activated, proliferate, and release potent cytotoxic molecules such as Granzyme-B, Perforin-1, and IFN-γ, which induce apoptosis (programmed cell death) in the targeted cancer cells. The activated CAR-T cells also contribute to an enhanced overall antitumor response by recruiting other immune cells to the tumor microenvironment. This targeted immunological approach aims to selectively destroy cancer cells while minimizing adverse effects on healthy tissues. Specific Claudin18.2 CAR-T cell products, such as CARsgen Therapeutics' CT041 (also known as satricabtagene autoleucel), have demonstrated promising efficacy and manageable safety profiles in clinical trials for advanced gastrointestinal cancers, including gastric and pancreatic cancer.
