CLEC12A-directed CAR-NK cells are an experimental adoptive cell therapy consisting of natural killer (NK) cells engineered to express a chimeric antigen receptor (CAR) targeting C-type lectin domain family 12 member A (CLEC12A), also known as CLL-1. CLEC12A is a cell surface glycoprotein frequently overexpressed on acute myeloid leukemia (AML) blasts and leukemic stem cells while being absent from healthy hematopoietic stem cells, making it a promising immunotherapeutic target. These CAR-NK cells typically incorporate intracellular signaling domains such as 4-1BB and CD3ζ to drive potent cytotoxic activity upon antigen recognition. Preclinical research suggests that the efficacy of CLEC12A-directed CAR-NK cells can be significantly enhanced by pretreatment with menin inhibitors, which upregulate CLEC12A expression on NPM1-mutated and KMT2A-rearranged AML cells.
