Overview

Clemizole is a first-generation antihistamine of the benzimidazole class that acts primarily as a histamine H1 receptor antagonist. It was originally marketed under the brand names Allercur and Histacur for the treatment of allergic reactions and pruritus but is no longer commercially available as an antihistamine[4][7]. Clemizole also exhibits activity as a serotonin receptor agonist, particularly at 5-HT2B receptors, which has led to its investigation for anticonvulsant effects in epilepsy syndromes such as Dravet syndrome and Lennox-Gastaut syndrome[4][6][7]. Its antiepileptic mechanism is believed to involve modulation of serotonin signaling rather than classic antihistaminic pathways[6][7][8]. Clemizole has also been shown to block TRPC5 ion channels and hERG potassium channels, with implications for cardiac safety[5]. The drug is being developed under the code name EPX-100 for rare epilepsies.
