CLM-022 is a unique, orally bioavailable synthetic pentacyclic triterpenoid derivative developed by Celloram and licensed to Genfit for global development in liver disease. It acts as a potent dual inhibitor of the priming and activation steps of the NLRP3 inflammasome, an intracellular protein complex central to innate immune responses and implicated in autoinflammatory disorders, metabolic syndrome, neurodegeneration, and resistance to cancer immunotherapy. By suppressing NLRP3 inflammasome activity, inhibiting pro-inflammatory cytokine production (such as IL‑1β), reducing pyroptosis (a lytic form of inflammatory cell death), and mitigating oxidation-induced cellular stress, CLM‑022 demonstrates strong anti-inflammatory and antioxidant properties. Preclinical studies show efficacy in models of acute liver injury (acetaminophen-induced) and systemic inflammation (LPS endotoxemia), with both oral and intravenous administration providing hepatic protection. The drug is being advanced primarily for acute-on-chronic liver failure (ACLF) but also has potential applications in chronic inflammatory diseases such as inflammatory bowel disease.
